Expert assistance with transfer from the community to a supportive care environment; including the coordination of moving and de-cluttering, cleaning and organizing services, site selection, application process, admission paperwork, property set-up and adjustment suppport: $2,500 - $10,000*
Expert assistance with transfer from hospital to skilled nursing facility; including selection, admission process, transfer support and room set-up: $500 - $1,000*
Expert assistance with discharge from hospital or rehabilitation facility to community; including discharge planning, home services set-up and verification and transfer support for 30 days: $250 - $1,000*
